Use the Youth in Arts form for all reservations.
Please duplicate form and use separate forms for each
performance reservation.
The fee is $8.50 per seat for students, teachers, parents and chaperons. Scholarship seating for children on the Free Lunch Program is $4.00 per seat.
If paying by check, please issue one check only, payable
to Youth in Arts. Include the school or individual name
on the check.
Payment must be received with
the reservation.
Reservation forms are dated and numbered as received,
and processed accordingly.
Confirmations will be mailed to school or individual
promptly.
